How do I reset my password?

To recover or change a lost password, navigate to the login screen of Eviti Connect and select Forgot Password. Once you fill out the form, an email will be sent to your associated address with a new, temporary password. Entering this on your next log-in will prompt you to change your account password permanently.

If you are already logged in, simply navigate to My Account in the menu, and select Change Password.

How do I update my provider data?

To update the information associated with your provider accounts, select My Account in the upper menu. From here, you can review and update contact information, NPIs, tax IDs, state licenses, passwords, and account recovery questions and answers.

What is a payer confirmation number? Where do I get one?

After entering a patient’s information, you may be prompted to input a payer confirmation number. This ensures that we send the treatment plan to the correct payer. Every payer has their own confirmation number—typically listed on a patient’s insurance card.
