Enter the treatment dates
After you verify the diagnosis details and answer any follow-up questions, we ask you for the treatment dates.

On the Enter Treatment Plan Dates
screen, enter the treatment plan start date.
The treatment end date automatically calculates based on the maximum cycle information. We add a three-week cushion to account for any delays that may occur during treatment.
- Update the dates if necessary, and then click Save and Continue.
The system analyzes the treatment plan.
If the treatment plan does not meet evidence-based guidelines, follow the steps in the Medical Review Required section.
