Attach additional documents

When our medical team reviews your treatment plan, they may email you to request additional documentation. To speed processing, you can attach the requested documents electronically instead of faxing or emailing them.

You can attach the following file types: .xls, .xlsx, .xlsm, .doc, .docx, .pdf, .ppt, .pptx, .csv, .jpg, .gif, .rtf, .bmp, and .txt.

  1. On the Eviti home page, click Saved Treatments in the top menu bar or click Review Saved Treatments in the left panel.
  2. Find your treatment plan in the list, and then click Attach Files in the Actions column. The treatment plan must be pending Eviti review.
  3. On the next screen, click Choose Files.
  4. Find and select the file or files on your computer or network. You can select multiple files.
  5. Click Upload & Save to attach the files to your treatment plan.

    Our medical team receives an email indicating that you attached the documents.

  6. Click Back to return to your saved treatments list.

To review the attached files, click Review Summary in the Patient column, or click Attach Files.
