The checklists include all the information that you need to enter a treatment plan into Eviti Connect.
- What is a payer confirmation number, and where do I get it?
The payer confirmation number ensures that we send the treatment plan to the correct payer. You can typically find the number on the patient's insurance card.
- Do I enter all the drugs in the chemotherapy treatment plan, or just the ones that need preauthorization?
Enter all drugs that the patient will receive during treatment.
- I'm having trouble entering clinical information. What should I do?
Ask a clinician at your practice to help you complete the treatment plan.
- I see a warning message after I choose a drug. What should I do?
Read the message carefully, and then click Accept to keep the drug that you entered, or click Cancel to remove it.
- I don't have the biomarker information that I'm being asked for. What should I do?
Enter Not Reported for the biomarker.
- How do I fix deviations in a treatment plan?
Review the deviation report, and then update the treatment plan. The goal is to correct any data entry errors so the treatment plan follows evidence-based guidelines, gets an Eviti code, and quickly gets approved by the payer.
- What should I do when a treatment plan needs medical review?

Wait for an email from Eviti with further instructions.

Log into Eviti Connect, click Review Saved Treatments, and then click Add Drugs.
